Requests for reasonable accommodations by University of Pennsylvania employees who have a disability (permanent or temporary) are to be made to the Office of Affirmative Action and Equal Opportunity Programs (OAA/EOP). To request assistance through the OAA/EOP, employees should contact the office at (215) 898-6993.
The Penn Access Transportation (PAT) vehicle is operated by the Department of Transportation and Parking and is available to transport members of the University's community with mobility and visual disabilities.
Authorization for use of PAT by faculty and staff must be obtained from the Office of Affirmative Action and Equal Opportunity Programs. Faculty and Staff must complete the Penn Access Transit Form and have their physician submit medical documentation to our office. You may fax the form to our office at 215-746-7088 or mail it to:
Office of Affirmative Action and Equal Opportunity Programs
3600 Chestnut Street
Sansom Place East, Suite 228
Philadelphia, PA 19104-6106
Students must receive authorization from the Office of Student Disabilities Services (SDS) at 215-573-9235 for further assistance.
Contents of requests are confidential and will only be shared as needed with the appropriate personnel to consider the implementation of a reasonable accommodation. Factors to determine whether a reasonable accommodation is necessary include the employee's request, the time the employee's request is made, what the essential job functions are, and contents of the supporting medical documentation. All requests are reviewed and assessed on an individual basis. If a change in job, medical condition or another accommodation is desired, it may be necessary to submit new documentation for review.
