Inquiry into an applicant's military service in the Armed Forces

Maiden name of a married applicant whose name has been changed by court order or otherwise

National Origin: Inquiry into languages applicant speaks, reads or writes fluently

Inquiry into language commonly used by applicant

Inquiry into how applicant acquired ability to read, speak or write a foreign language. Inquiry into applicant's:

  5. Parentage

List all organizations of which you are a member which you feel will be an asset to you in this position (do not list any group which may reveal your race, religion or nationality)

Inquire into applicant's membership in the Communist Party or any organization on the Attorney General's list

List all clubs, societies and lodges to which you belong
Photograph:  

Requirement that an applicant for employment affix a photograph to his employment application form

Request an applicant, at his option, to submit his photograph

Inquiry into an applicant's religious denomination, religious affiliations, church, parish, or religious holidays observed

An applicant may not be told: "This a Catholic, Protestant, or Jewish organization."

Inquiry as to whether applicant regularly attends a house of worship

Sexual Orientation:   All inquiries into an applicant's sexual orientation
Note 1:   After Hiring: Medical examination forms, group insurance, census and enrollment cards and surety bond application forms, containing inquiries relating directly or indirectly to race, color, religion, national origin, ancestry or sexual orientation may not be filled out prior to an applicant's hiring for employment, but are permissible after hiring
Note 2:   Security: Where an employee is under contract with a goverment agency which requires information relative to applicable security regulations, it shall be proper for agency to ask only such questions as are necessary in order to obtain the required security information.
