Pre-Tax Expense Accounts: September
Reimbursements from the Health Care and
Dependent Care Pre-Tax Expense Accounts for expenses incurred
for the Plan Year July 1, 2002 to June 30, 2003 can
be submitted until September 30, 2003. Requests can
be sent via fax or mail, as listed below:
Claims Processing, P.O. Box 1853,
Alpharetta, GA 30023-1853
Fax: (678) 762-5900
These reimbursements are based on
receipts for expenses incurred between July 1, 2002 and June
30, 2003. Expenses are considered incurred on the day
the service is provided, not when you are billed or when the providers
are paid. Keep in mind that for services partially covered by
a health plan, you must submit an Explanation of Benefits (EOB)
along with your reimbursement request. If you have not received
your EOB from your provider by the September 30 deadline, you
must still file your claim by September 30, and then forward
the EOB when you receive it. Note on your claim form that
you are waiting for your EOB.
If you have a Pre-Tax Expense Account, you
can download reimbursement forms and view your account balance
and activity by logging on to www.pennbenefits.upenn.edu.To
speak to a customer service representative about your account,
call the Penn Benefits Center at 1-888-PENNBEN (1-888-736-6236)
