Improving long-term care
The first facility of its kind in the country, the NewCourtland Center for Transitions and Health at Penn's School of Nursing is devoted to understanding the unique needs of patients suffering from such chronic illnesses as heart failure, diabetes and depression.
Founded in 2006 and directed by Penn Professor of Gerontology Mary Naylor, the center is a hub for research, education and policy in the emerging field of transitional health, in which researchers study what happens to patients as their health condition changes due to chronic illness. With a new $5 million endowed gift from NewCourtland Elder Services, the center will research transitions in health among the chronically ill and the effects of those transitions on caregivers, spark innovations that can ease health transitions, and promote evidence-based practices that can inform health care policy.
