Access Request Forms

Before you can access data in the University Data Warehouse, you must have both an Oracle account and password and authorization to use the Data Warehouse. If you plan to use Business Objects as your query tool, you will also need authorization to use that software.

To request authorization and access, you must submit a Data Warehouse Access Request Form for each data collection you need. Be sure to check the appropriate check box on each form if you plan to use Business Objects or InfoView only. Select the form from these data collections:

• Advancement Data Collection (information from ATLAS)
• Assets Data Collection (property management asset information from the BEN Assets module of BEN Financials)
• Faculty Data Collection (information from the Faculty Information System) in Acrobat (.pdf) format.
• General Ledger , Salary, Position Inventory and PennERA Proposals Data Collections in Acrobat (.pdf) format.
  (Note: You may also this form to request access only to EMPLOYEE_GENERAL and related tables, for the purposes of viewing basic employee and primary appointment information with no salary or job details.)
  • Sponsored Projects version of the financial collection form (used only for access to old Sponsored Projects data, not Proposals)
• ISC Billing Data Collection (access to Network billing, Telecommunications, and Mainframe Chargeback data) in Acrobat (.pdf) format.
• Learning Management Collection (information from Knowledge Link)
• Space@Penn Data Collection in Acrobat (.pdf) format.
• SSN Access Form in Acrobat (.pdf) format.

• Student, Course and Instructor Data Collections in Acrobat (.pdf) format.
  • BRS Data Collection form, used only if you do not already have access to the Student data collection. In Acrobat (.pdf) format
• University Payroll/Personnel Change Log data in Acrobat (.pdf) format.
  (Note: Access to the Salary Management data collection is the required prerequisite for access to Change Log data.)



• BEN Reports in Acrobat (.pdf) format.

To read these forms you will need Netscape 2.x or higher and the Acrobat reader. You can download Acrobat Reader software from Adobe System's web site free of charge.
